This Creative Release Form ("Release") grants Marked4Liife Productions the right to use photographs and/or video footage captured of the undersigned ("Model/Client") for promotional, commercial, or educational purposes.

1. I authorize Marked4Liife Productions and its representatives to use, publish, and distribute media in print, digital, and social formats.

2. I waive any right to inspect or approve final materials where my likeness appears.

3. I release Marked4Liife Productions, its employees, and assigns from all claims related to the use of my image or likeness.

4. This Release is irrevocable and binding.

5. **Scope of Work:** Videographer will provide filming and editing services as outlined in the client’s project plan (e.g., wedding, event, short film).

6. **Payment Terms:** A 50% non-refundable deposit is required to reserve the date. Balance is due upon final delivery. Accepted payments: Cash App, Zelle, Square.

7. **Editing & Revisions:** Two rounds of edits are included. Additional revisions may incur a $50 fee per round.

8. **Delivery:** Final edited film will be delivered within 2–4 weeks unless otherwise stated.

9. **Rights & Ownership:** Marked4Liife Productions retains the copyright of all footage. Client is granted personal or promotional use rights. Raw footage is not included unless purchased separately.

10. **Liability:** Videographer is not liable for conditions beyond reasonable control (e.g., weather, acts of God, equipment malfunction).
